John R. Kozlowski, DDS began his journey at SUNY-Buffalo, where he earned a bachelor's degree in sociology. He later earned his D.D.S. from SUNY-Buffalo School of Dentistry in 1970 and obtained his certificate in pedodontics from Children's Hospital two years later. From there, he became the Chief of Pedodontics and Captain in the United States Army until 1974. James M. O'Leary, DMD is a lifelong resident of Waterford, Connecticut. He attended Jordan Elementary, Clark Lane Middle and Waterford High School. He received his B.S. degree from The Ohio State University and his DMD degree, Summa Cum Laude from the Tufts University School of Dental Medicine in Boston. While at Tufts, he also completed a comprehensive 2-year specialty training program in orthodontics. In July 2007, Dr. O'Leary became board certified by the American Board of Orthodontics. He has been a member of Orthodontic Associates since 1987.
Dr. O'Leary is committed to providing his patients with the highest quality, modern orthodontic care. He is an active member in the American Association of Orthodontics, and he is also past President of the New London County Dental Society. Gregory A. Hack, DDS is a graduate of Brown University and the Columbia University School of Dental and Oral Surgery. Following the completion of a one-year general practice residency at Lenox Hill Hospital in New York City, Dr. Hack pursued his orthodontic specialty training at the University of Connecticut Health Center. Dr. Hack then joined Orthodontic Associates in October 1991.

Dr. Jeff Serff
Jeff Serff is a graduate of Brookstone School in Columbus, Georgia. He earned his bachelors of science degree in Microbiology from the University of Georgia and graduated with honors from the Medical College of Georgia, School of Dentistry in 2000.

Dr. Ross Stryker
After completing his undergraduate studies at Kansas State University, Dr. Ross Stryker attended the University of Missouri-Kansas City School of Dentistry and graduated in 1982. He served his country in the military for 12 years; eight as a general dentist. He received his orthodontic training in the military from 1990 to 1992 and served his last four years as an orthodontist.

Dr. David E. Paquette
He is married to Jenny and together they have four beautiful girls, the twins Samara and Seriann, their younger sister Delaney and the youngest Norah. He has been in orthodontic practice in Charlotte, NC since 1989.
Dr. Paquette graduated in 1979 from UNC- School of Dentistry with a Doctorate of Dental Surgery. He received a Master’s in Pediatric Dentistry from UNC in 1983. His Master’s thesis won the American Academy of Pediatric Dentistry Graduate Research Award that same year. Dr. Paquette is board certified by the American Board of Pediatric Dentistry and served as a consultant in Pediatric Dentistry for the United States Air Force in Europe and Asia until 1987. He received his orthodontic training at St. Louis University Department of Orthodontics where he obtained his Master’s degree and specialty certificate in orthodontics in January 1989. His Master’s thesis in orthodontics won the coveted Milo Hellman award in 1990. It is presented annually to only one orthodontist worldwide. In addition, Dr. Paquette is also board certified by the American Board of Orthodontics.
Dr. Paquette has written and published multiple articles for scientific journals as well as contributed chapters to two orthodontic textbooks. He is frequently requested to lecture both domestically and worldwide. He is actively involved in ongoing research as a consultant to leading orthodontic products manufacturers. He is the past president of both the Charlotte Dental Society and the NC Association of Orthodontists. He has been involved with Smile for a Lifetime since 2009.

Stephen Burke is our hometown doctor. Born and raised in the Dayton area, he graduated from Carroll High School in 1984. He received his BS degree, magna cum laude, from the University of Dayton as a member of the Honors Program. Daniel S. German, DDS
Since 1987, Dr. Dan has been providing care for patients in his Dayton, Ohio private practice. He completed his undergraduate and dental school education at Case Western Reserve University, where he received honors including a research fellowship, followed by his orthodontic residency education at the University of Louisville. Dr. Dan German currently serves as a part time Clinical Assistant Professor at The Ohio State University.
Dr. German has been teaching innovative orthodontic therapy as the keynote speaker at many Orthodontic/Dental meetings ranging from Alaska to South Africa. At last count, he has delivered more than 30 such oral presentations. He has published in the Journal of the American Dental Association, American Journal of Orthodontics and Dentofacial Orthopedics, and The Journal of Clinical Orthodontics.
Dr. German was a recipient of the Humanitarian Award by the Ohio Dental Association for his charitable work with the homeless in need of dental and medical care. He was a founding member of Dentistry for Friends in Need, Inc. His organization provided care for the homeless in the St Vincent Hotel in Dayton, Ohio during the early 1990’s.
Dr. German has been providing orthodontic care to children for over twenty years, and STILL loves kids and orthodontics! His wife Teri is an Indiana native and Indiana University graduate that has finally caved in and purchased Ohio State University Buckeye attire for their seven children. Dan’s late father, Bill, played football for The Ohio State University under Coach Woody Hayes.
Dan likes to spend free time and vacations with his family, making an annual drive out west to play in the mountains. As a fitness buff, Dan has been spotted running extremely long distances, such as a 50-mile race in the mountains and a "60 mile jog” from Xenia to Covington, Kentucky to raise funds for the American Cancer Society.

Dr. Jenny R. Maple
A native of Toledo, Dr. Maple is a graduate of The Ohio State University Graduate Orthodontic Program where she received her MS and Orthodontic Certificate. She also completed her dental training at OSU where she graduated Summa Cum Laude. She received several awards and honors during her dental education and is happy to have joined Associated Orthodontics shortly after graduation from her program in July, 2003. Dr. Maple is also the Founder of the Columbus Chapter of the Smile For a LifeTime Foundation.
Dr. Maple and her husband, Steve, have lived in New Albany since 1997. They have one son, Tommy, who was born in 2007.

Dr. A.G. Burris
Dr. A.G. Burris has been providing orthodontic care in the Lowcountry for over 30 years. He received his DDS from the University of Tennessee, served as valedictorian of his graduating class, and completed his residency @ UT College of Dentistry in Memphis. Tennessee
Dr. Joel Gluck
Dr. Joel Gluck has practiced orthodontics since 1983 when he opened his own practice in Nashville. He earned his undergraduate degree at Washington University in St. Louis, and his dental degree at the University of Tennessee. Dr. Gluck then completed an orthodontic residency at the University of Michigan, where he also received a Master of Science degree. In 1993, he became board certified and is a Diplomate of the American Board of Orthodontics. Dr. Gluck has been an Assistant Clinical Professor at Vanderbilt University Medical School, Division of Orthodontics, as well as on the hospital staff at Vanderbilt Hospital.
Becoming a Diplomate of the American Board of Orthodontics is a unique achievement. In order to become board certified, Dr. Gluck passed a comprehensive oral and written examination covering all phases of orthodontic and dentofacial orthopedic care. He also had to present to the Board detailed case reports on the treatment he provided for a broad range of orthodontic problems.
Attending continuing education courses each year enables Dr. Gluck to stay at the forefront of his specialty so that he can provide the best care for his patients. He keeps up with the latest advancements in orthodontics by maintaining memberships in the American Association of Orthodontists, World Federation of Orthodontists, American Dental Association, Southern Society of Orthodontists, College of Diplomates of the American Board of Orthodontics, and numerous national study clubs.
Dr. Gluck and his wife, Linda, are the parents of two sons and two daughters. They enjoy many outdoor activities, as well as being involved in numerous civic organizations and charities.

Dr. Daniel Sawrie is a Chattanooga native and attended the Baylor School where he graduated in 1991. He received his undergraduate degree from the University of Tennessee, Knoxville and then attended the University of Tennessee College Of Dentistry where he earned his D.D.S. degree. Dr. Brett R. Coleman
Dr. Coleman is a native of North Ogden, Utah. He attended Weber High School and Weber State University. Following his undergraduate training at Weber State, he attended the University of Louisville School of Dentistry graduating Summa Cum Laude with a DMD degree. He was inducted into the Alpha Omega International Honorary Dental Fraternity for attaining the highest academic standing during his years of study.
