
If you or someone you know could benefit from orthodontic treatment, but the finances are keeping you or them from achieving this dream, the Smile for a Lifetime Program may be beneficial for you. To find out if the Smile for a Lifetime program is available in your area and if you qualify, you first need to locate an orthodontist in your area that participates in the Smile for a Lifetime Foundation. Once you have done this and reviewed the requirements needed to qualify for this program, please view the instructions below on how to apply for treatment.
All interested patients must be residents of the area of the foundation that serves you. To apply for the Smile for a Lifetime program, applicants must complete and supply the following:
- Smile for a Lifetime Foundation Application
- A letter of recommendation.
- A photograph of the applicant’s teeth. (Teeth must clearly be visible.)
Please contact your participating orthodontist for an application and return the completed application, letter of recommendation and photograph to their office. The Board of Directors for your orthodontists’ local chapter will meet quarterly to make their selections.
